Over the last month, HealthTrust sent the following notices to our Members or Enrollees:

• IRS Extends Deadline for 1095 Forms to Individuals. On December 4, an email was sent to Member Groups offering HealthTrust medical coverage announcing a change in the IRS Reporting Deadlines for the 2018 calendar year. The IRS extended the deadline for employers to furnish ACA Forms 1095-B and 1095-C to individuals from the original date of January 31, 2019 to March 4, 2019. The Notice expressly DOES NOT extend the due dates for filing the Forms 1094 and 1095 Bs and Cs with the IRS, which remain February 28, 2019 (if on paper) and April 1, 2019 (if electronically). • Important News About Your Benefits for 2019. On 12/10/18, documents were mailed to certain HealthTrust Enrollees of Member Groups with a January renewal. The documents notified Enrollees of enhancements to their medical plan (where applicable, depending on their coverage plan). The documents also shared information about new programs and services. For a copy of the document sent to your Enrollees, please contact HealthTrust Enrollee Services.

• HealthTrust Financial Statements Now Available. On 12/18/18, a postcard was mailed to CEOs of all HealthTrust Member Groups. The postcard notified them that the FY2018 Audited Financial Statements, FY2018 Capital Adequacy Reserve Actuarial Evaluation and FY2018 Claims Reserves Actuarial Report were all available on the HealthTrust website. • Announcing the Retirement of Martha Swasey. On 12/19/18, an email was sent to all Member Groups, announcing the retirement of Martha Swasey, HealthTrust Member and Enrollee Services Manager, who retired on January 4. Click the link to read Martha Swasey's Retirement Email.

• New Benefit Program – CVS Caremark Transform Diabetes Care®. On 12/20/18, HealthTrust sent emails to all Member Groups offering medical plans that include CVS Caremark Prescription Drug coverage. The email announced a new Diabetes Management Program, CVS Caremark Transform Diabetes Care, that became available on January 1, 2019 to Enrollees who currently take prescribed diabetes medication. • SmartShopper Rewards the Entire Family. At the end of December a postcard was mailed to Enrollees in HealthTrust medical coverage (except for those enrolled in a Medicomp Three plan), reminding them to use SmartShopper to find cost-effective medical services. • Slice of Life 2019 Journey Guide. At the end of December, Onlife Health mailed the 2019 Slice of Life Journey Guide to all eligible Enrollees. The guide provides details and instructions for participating in the program. Click the link to read the 2019 Slice of Life Journey Guide.

• $2 million+ : Net HealthTrust medical plan cost savings through the SmartShopper program from January through December 2018. Please remind your employees to “look before they book,” by using SmartShopper before their appointments to find cost-effective facilities for their medical services.

• 1,114: Total number of participants in the Health for the Holidays challenge from mid-November to mid-December 2018. Half of participants earned $15 toward their Quarter 4 Personal Wellness Journey Reward. 94 percent of participants who replied to a post-challenge survey said they enjoyed the challenge and 68 percent said the challenge helped them improve their habits. Congratulations to SAU #57 with the most employees who earned the reward.

• 94%: Satisfaction rate with the Slice of Life 2018 Health Coaching program, based on an end-of-year survey. Here are just a few comments from participants:
